Fifty y/o female with elevated PTH and clinical suspicion of hyperparathyroidism underwent a double phase and substraction 99mTc sestamibi scintigraphy.  At 10 min the thyroid gland is well visualized. At 2 h the gland washes out almost completely. In the 99mTc perthecnetate image it is seen a homogeneous uptake in the thyroid gland, concordant with the sestamibi 10 min scan. In addition a right oblique view was obtained.
Tc 99m parathyroid scintigraphy is a sensitive (90%) and specific (90%) tool for the diagnosis of adenoma, hyperplasia and carcinoma of the parathyroid glands.
